Health Screening Day is 11/02/2023
Tenth grade students and new incoming students will have their vision & hearing screenings done.
Please remind you child/children to wear their contacts, glasses & hearing aids if they have them.



Meningococcal Conjugate (MenACWY) Vaccine

Effective July 1, 2021, a minimum of 2 doses of MenACWY vaccine. The first dose should be administered prior to entering 7th grade. The final dose should be administered prior to entering 12th grade. 
**All rising seniors will need this vaccine after their 16th birthday in order to enter the 12th grade**
A printed immunization form signed by a medical professional must be submitted to the JFHS clinic.


Please make an appointment with the nurse to bring any necessary supplies or medications to the clinic.

All children should be fever free for 24 hours (without the use of fever reducing medications) before returning to school.

Students may carry cough drops with them but SHOULD NOT share with other students.


THOROUGH HAND WASHING IS THE BEST DEFENSE AGAINST GERMS!!


Please Remember:

  • If your child/children are going on an overnight field trip, make sure to have the Medication Administration form filled out for any medications they need while on their trip. A form is required for each medication. Medications must be given to the clinic 2 days prior.
  • Please have the appropriate medical forms on file in the clinic if your child/children have medical conditions.
  • All medication forms expire each school year, including the acetaminophen and antihistamine forms.
  • Please register your child/children School Care. This is our electronic charting system that notifies you of treatment provided to your child/children. School Care is HIPAA & FERPA compliant.
